If a medal in your pack of 20 works poorly, do you replace it?

Of the 20 medals in a pack, there is no reason one particular medal should work poorly.

But if that were nevertheless the case, the cause would be a faulty QR code or a defect during laser engraving.

In that case, we will of course replace the medal.
